Qualifications

• Minimum 2 years in human services.
• Experience with homelessness, HIV, substance abuse, mental illness, and entitlements.
• Familiarity with HIPAA and confidentiality.

Responsibilities

• Carry caseload and meet clients weekly for assessments with supervisor.
• Link clients with providers and resources across health and human services.
• Escort clients to scheduled psychiatric/medical care.
• Maintain audit-ready client records and progress notes; develop service plans.
• Ensure safe, client-centered, timely, equitable care during crises.
• Organize and facilitate on/off-site activities and group sessions.
