Job summary

COMHAR is a nonprofit community-based organization delivering health and human services. We have an immediate opening for a Care Coordinator in our Children Services department to coordinate services for youth in the Intensive Behavioral Health Services (IBHS) program.

The role collaborates with families, schools, and providers to arrange evaluations, referrals, and placement in appropriate services. A high school diploma with college credits and a related degree are required, with bilingual

Qualifications

• High school diploma with 12 college credits in behavioral health/human services.
• Bachelor’s degree in social work, counseling, psychology, sociology, gerontology, counseling education or related field.
• At least 1 year of experience in health or human services; behavioral health or substance use field preferred.
• Driver's license required.

Responsibilities

• Meet with families for intake and coordinate services.
• Schedule and confirm evaluation/re-evaluation appointments.
• Provide information on Medication, Outpatient, Neurology, etc.
• Attend Biopsychosocial Evaluations/Re-evaluations.
• Arrange referrals and placements to Partial Hospitalization, Residential, and inpatient programs.
• Complete packets for Community Behavioral Health.
• Respond to CBH issues and follow up on submissions.
• Home visits as needed.
