Job summary

Equitable Social Solutions in Oregon seeks a program coordinator/case manager to coordinate resources and care for persons enrolled in the program. You will manage individual client services to meet needs and intervene to resolve issues when goals or service delivery are not met.

The role includes developing intake, progress and discharge reports; scheduling interdisciplinary case conferences with partners and family members; arranging equipment; contributing to program evaluation; and upholding

Qualifications

• Bachelor's degree or equivalent experience preferred.
• Crisis management ability
• Strong interpersonal, verbal, and written communication skills
• Proficient in Microsoft Office and data entry
• Able to work independently and as part of a team

Responsibilities

• Coordinate resources and care for persons enrolled in the program.
• Manage the provision of individual client services, ensuring clients' needs are met.
• Intervene and problem solve issues if outcomes/goals and service delivery are not met.
• Develop intake, progress and discharge reports; track and document status of person served.
• Schedule/interdisciplinary case conferences with community partners, funders and family members.
• Arrange for provision of equipment to enable achievement of goals.
• Participate in program evaluation and quality assurance.
• Maintain confidentiality and HIPAA compliance; adhere to ethics policies.
• Attend and participate in staff development and continuing education as required.
• Perform other duties assigned.
