Resources for Human Development (RHD) is seeking a compassionate Case Manager with a CADC credential to join our Lehigh Valley ACT Program in Bethlehem, PA. This full-time role focuses on delivering recovery-oriented, community-based behavioral health services within a multidisciplinary team.

You will help participants navigate services, access housing, education, healthcare, and other supports while advancing your clients toward independence and improved quality of life.

Qualifications

• Current CADC certification is required.
• Two years of experience in behavioral health, addiction recovery, homelessness services, or BHH/IDD programs.
• Intermediate computer skills and ability to complete electronic documentation.
• Strong interpersonal, communication, and customer service skills.
• Valid driver's license and ability to travel throughout the community.

Responsibilities

• Work as part of a multidisciplinary ACT team providing holistic, recovery-focused services to participants.
• Assist individuals in developing and achieving goals outlined in their recovery plans.
• Connect participants to community resources, educational opportunities, healthcare services, housing supports, and recovery programs.
• Advocate for participants to ensure access to needed services and supports.
• Conduct substance use screenings and support participants in achieving recovery goals.
• Utilize CADC knowledge and skills to promote recovery, relapse prevention, and treatment engagement.
• Coordinate referrals to substance use treatment providers, recovery services, and community supports.
• Develop and maintain relationships with community agencies and service providers.
• Provide community-based services throughout the Lehigh Valley service area.
• Transport participants to appointments and community activities when needed.
• Complete assessments, service documentation, and reports in accordance with organizational and regulatory requirements.
• Identify service gaps and recommend strategies to enhance participant outcomes.
• Support participants in achieving greater independence, stability, and quality of life.
