Job summary

BI Incorporated is seeking a Case Manager to provide intensive supervision and individualized support to ISAP participants. You will develop service plans, conduct intake and orientation, and coordinate with translation and community resources.

Responsibilities include monitoring, documentation, home visits, and emergency counseling. Minimum qualifications include a high school diploma or GED, 2 years of customer service or case management experience, and the ability to work in Spanish.

Qualifications

• High School Diploma or GED required; Bachelor's degree preferred.
• Two years of customer service or case management experience required.
• Bilingual Spanish is required.

Responsibilities

• Develops, implements, coordinates, reviews and updates case management and individual service plans for ISAP participants.
• Meets with participants face-to-face regularly as required by contractual obligations.
• Maintains accurate case records and ensures confidentiality.
• Conducts intake interviews and orientation for ISAP participants.
• Installs electronic monitoring equipment and monitors compliance.
• Assists participants with obtaining travel documents and maintains logs.
• Evaluates English proficiency and coordinates translation services.
• Connects participants with community resources including transportation, medical, legal and other services.
• Supervises participants in BI offices and coordinates schedules for counseling and court appearances.
• Reviews plans, conducts risk/needs assessments and writes progress reports.
• Reports program violations and performs home visits to verify residences.
• Provides emergency counseling and maintains translation resources.
• Performs other duties as assigned.
