Job summary

North Central Mental Health Services, Inc. is seeking a Behavioral Health Case Manager to empower lives and build futures in Franklin County. The role connects clients to essential services and coordinates care, while advocating for dignified, integrated treatment.

You will develop personalized care plans, monitor progress, and collaborate with families and professionals to support independence and recovery. Training and growth opportunities accompany a supportive team.

Qualifications

• Bachelor’s degree in social work, psychology, behavioral health, or related field.
• Compassion and dedication to make an impact.
• Organized and detail-oriented; manage multiple resources.
• Familiarity with community resources (preferred).
• Prior experience in mental health or case management (preferred).
• Ability to provide 748 patient service hours annually (62.50/month).
• Valid Ohio driver’s license and reliable transportation.
• Completion of 90-day paid training program.
• Successful background check and pre-employment drug screening.

Responsibilities

• As Case Manager, connect clients to healthcare services and community resources.
• Care Plan Development with clients, families, and professionals.
• Service Coordination for therapy, housing, employment, and recovery programs.
• Advocacy across healthcare, legal and social service systems.
• Ongoing Monitoring of progress and plan adjustments.
• Crisis Intervention to ensure client safety.
• Education and Support on mental health and resources.
• Collaboration with professionals and benefit providers.
• Documentation and Compliance in the EMR system.
• Empowerment and Cultural Sensitivity in services.
