Job summary

San Gabriel/Pomona Regional Center seeks an experienced nurse to coordinate medical services for clients, staff, and families. You will conduct assessments, manage care plans, and ensure high quality health services and compliance across programs.

You will educate staff and partners, collaborate with medical consultants, and support quality initiatives while serving in a regional community setting with travel as needed.

Qualifications

• BSN required.
• RN license required.
• Public Health Nursing certificate required.
• Four years of progressively responsible experience in public health nursing or equivalent experience.
• Master’s degree can substitute for one year of experience.

Responsibilities

• Provide nursing coordination and consultation to Regional Center staff, service providers, clients, and families regarding health and medical needs within the nursing scope of practice.
• Offer information on medical terminology, disease processes, procedures, and health topics to support care planning.
• Conduct evaluation visits and develop comprehensive nursing assessments for levels of care and facility placement.
• Support, monitor, and prepare monthly ARFPSHN reports to ensure compliance and quality of care.
• Coordinate with SG/PRC Medical Consultant and Client Consultation Committee for communication and decisions on health services.
• Create educational presentations for staff, parents, providers, and community groups on health topics.
• Coordinate with QA Unit and Placement Committee to verify care quality and appropriateness of facilities.
• Perform nursing death reviews to assist M&M Committee in care evaluation.
• Coordinate with local health care providers and hospitals for client health needs.
• Act as an Executive Director Designee providing consents for medical and dental procedures.
• Perform other duties as assigned to support organizational goals.
