Pinnacle Treatment Centers, Inc. is seeking a full-time Counselor in Kern County, CA. You will orient new patients, perform intakes and assessments, and develop individualized treatment plans to support recovery.

Qualifications

• Possess an associate degree in relevant field and credentials to provide counseling services.
• Kern County requires a bachelor’s degree or high school diploma/GED with 75% certification education.

Responsabilités

• Provide orientation to new patients on program rules and patient rights.
• Conduct intake, discharge planning, initial assessments, and treatment plans within guidelines.
• Involve patients in assessment and treatment planning to support recovery.
• Administer biopsychosocial assessments at intake and during treatment.
• Develop individualized treatment plans using SNAP to meet short- and long-term goals.
• Monitor patient conditions and address relapse indicators early.
• Comply with QA and Clinical Risk Management policies, including high-risk screening tools.
• Provide counseling and referrals to necessary services and support groups.
