Volunteers of America Delaware Valley is seeking a dedicated outreach/CARE coordinator to work with the IMPACT team in designated communities. As part of the IMPACT team working side by side with law enforcement in designated communities, responsibilities include the coordination of care management by providing information to vulnerable populations in various settings throughout the community, assessing the emergency needs of each individual requesting service; providing immediate, direct services, and to work as part of the team for effective care management. Provide highly individualized services in the community and promote client self-determination and decision-making. Provide essential expertise and consultation to the team to promote a culture in which each client's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities.

Credentials/Abilities:
The employee in this position must have a High school diploma or equivalent with a minimum of two years work experience in a relevant field. Must have direct experience in working with homeless population and individuals who suffer from substance abuse and mental health issues. Must possess effective written and verbal communication skills and an ability to interact appropriately with clients. Must be able to pass a criminal background check. Valid driver’s license required and the ability to meet agency driving standards. Bilingual is a plus.
