Outreach is seeking a LEAD Center Case Manager in Scranton, PA to deliver direct case management for Workforce Development participants. This full-time role carries a standing caseload of 20–25 active participants and focuses on career planning, job readiness, placement, and follow-up.

You will coordinate wraparound services with Dress for Success Lackawanna and related Outreach programs, maintain timely case notes, and report to funders as required.

Formación

• Bachelor’s degree in a human services field or related discipline.
• 2–3 years of case management/workforce development experience.
• Ability to handle data entry and reporting for funders.

Responsabilidades

• Manages a caseload of 20–25 active participants and supports employment goals.
• Delivers life skills and employment readiness workshops to individuals.
• Develops and monitors individualized service plans and tracks progress.
• Coordinates referrals with Reentry, GED, and other Outreach programs.
• Maintains accurate case notes and timely documentation for reporting.
• Supports Dress for Success Lackawanna operations as needed.
