Job summary

United Neighborhood Centers of NEPA (UNC of NEPA) is seeking a Case Manager in Scranton, PA to provide comprehensive case management and supportive services to individuals and families in Community Services programs. You will assess needs, set goals, and connect participants with housing options and resources to promote stability and independence.

The role requires strong communication, organization, and data-entry skills, with the ability to work with diverse populations.

Qualifications

• High school diploma or equivalent required.
• Bachelor's degree in social work, human services, counseling, psychology, sociology, or a related field is required.
• Previous case management experience preferred.
• Experience with diverse populations and barriers to housing or self-sufficiency preferred.
• Experience with electronic case management or client data systems preferred.

Responsibilities

• Conduct participant intake, eligibility screening, and needs assessments in accordance with program requirements.
• Maintain an assigned caseload and provide ongoing case management and supportive services.
• Collaborate with participants to establish goals and action plans for stability and self-sufficiency.
• Assist participants in identifying and securing housing and community resources.
• Connect participants with employment, education, childcare, healthcare, mental health, transportation, and other services.
• Manage budgeting, financial literacy, and life skills development as appropriate.
• Maintain participant files, documentation, and data in required systems.
• Complete program reports to meet grant, contract, and funder requirements.
