Job summary

Beth Israel Lahey Health is seeking a dedicated Physical Therapist for our Home Health team serving Peabody and Lynnfield, MA. The PT develops and directs in-home care plans, coordinating with families and physicians to meet patient goals.

Requirements include a Massachusetts PT license, DPT preferred, and 2+ years of PT experience (home health preferred). BLS certification is required, with OASIS experience preferred. Competitive salary and comprehensive benefits offered.

Qualifications

• 2+ years of physical therapy experience, preferably in home care/public health.
• Massachusetts Physical Therapist license required.
• Active BLS certification required.
• OASIS experience preferred.

Responsibilities

• Coordinate care for assigned patient caseload and supervise PTA staff if applicable.
• Complete initial patient assessment and develop a patient-specific plan of care with patient and family.
• Initiate and revise the plan of care; administer therapies as prescribed.
• Communicate with physicians regarding patient needs and obtain orders as required.
• Maintain HIPAA confidentiality and complete timely OASIS documentation.
