Beth Israel Lahey Health at Home at Hamilton, Ipswich & Wenham, Gloucster and surrounding communities is seeking a part-time Physical Therapist to coordinate home-based patient care in MA. Qualifications

• Physical Therapist with current licensure to practice therapy in the state of Massachusetts.
• Active American Heart Association BLS required; if not, BILHAH BLS competency.
• Graduate of an accredited Physical Therapy program; DPT preferred.
• OASIS experience is preferred.
• Minimum 1–2 years PT experience; public health or home care preferred.
• Must be a licensed driver with a insured vehicle.

Responsibilities

• Coordinate patient care for assigned caseload; delegate to PTA staff and supervise care.
• Complete ante physical assessment and patient history.
• Develop patient-specific plan of care with goals and therapeutic actions.
• Includes patient/family in planning; revises plan as needs change.
• Administer medications/treatments as prescribed; counsel patient/family.
• Instruct, supervise and evaluate home health aide care every two weeks.
• Coordinate with physician and other team disciplines to ensure care plan.
• Identify discharge planning needs and coordinate prior to discharge.
• Attend case conferences and team meetings; communicate with PTA and other providers.
