Job summary

Community Council Health Systems in Philadelphia is hiring full-time Case Managers to assist individuals with mental illness and families, ensuring access to housing, medical care, education, and employment. You will deliver case management in line with consumer plans, collect notes, and coordinate services across settings, including telehealth when needed.

The role requires a bachelor’s degree in a related field and at least one year of direct human services experience; a valid driver’s license

Qualifications

• Bachelor’s degree in relevant social services fields.
• Minimum of one year experience in human services with direct client contact.
• Two years paid experience in public or private human services, with one year in direct consumer contact.

Responsibilities

• Assist consumers or families in obtaining culturally appropriate basic living needs (housing, food, medical care, recreation, education, employment).
• Provide services within consumer and family culture.
• Provide case management per consumer-specific service plan with goal-oriented outcomes.
• Conduct services at consumer’s location or Case Management office when appropriate.
• Maintain regular contact schedule (weekly to monthly) based on assessment.
