Job summary

Qualifications

• Bachelor’s degree or higher in a relevant field.
• 1–2 years experience in health care, non-profit, or community-based organizations serving low-income communities.
• Nonclinical role requiring strong follow-through and data-oriented tasks.

Responsibilities

• Identifies, outreaches, and enrolls patients eligible for the IMPaCT intervention, ensuring enrollment goals are met weekly and annually.
• Uses the electronic health record to identify and produce reports of patients meeting eligibility criteria for CHW support.
• Supports patients in creating a health-related goal to work on with the CHW.
• Ensures accurate and timely documentation in specified platforms including REDCap.
• Provides support for IMPaCT quality improvement and evaluation initiatives, including participation with assessment, data entry, cleaning, and reporting.
