Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for Family Medicine in Chicago, IL. The role works under the supervision of collaborating physicians to deliver outpatient primary care within a busy clinic setting.

Responsibilities include taking patient histories, conducting exams, ordering tests, interpreting results, and managing treatment plans. Strong commitment to evidence-based practice and team collaboration is essential for improving patient outcomes.

Qualifications

• Licensed APN or PA in IL with DEA and controlled substances license.
• Master's degree in family medicine or related specialty.
• BLS/CPR certification required.

• Under the direction of collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Maintains records and writes progress notes on status, treatments and procedures.

• Full Time: 40 Hours/Week
• Hours: Between the hours of 7am - 6pm. Rotating early/late days
• Rotating call/weekends
• Outpatient
• Exciting opportunity to join Endeavor Health in the Primary Care - Family Medicine Department.
• Under the direction of the collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions.
• Maintains records and writes progress notes on status, treatments and procedures.
• Follows patients through their course of treatment in the clinic and hospital setting.
• Collaborates with physicians and the health care team to achieve optimal patient outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice.
• Participates in the development, implementation, and evaluation of standards of practice in the specialty.
• Assesses the learning needs of patients and families across the continuum.
• Employs strategies, interventions and teaching to promote, maintain, and improve health.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities.
• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
• Supports the strategic plan of the organization.
• Adheres to all Endeavor Health policies and procedures.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA experience preferred
