Job summary

Inova Fairfax Hospital is seeking an Advanced Lung Disease Coordinator to join the Transplant team. The role focuses on coordinating care for ILD patients across outpatient settings, including diagnostics, education, referrals, and monitoring.

It is a full-time day-shift position with occasional on-call remote requirements. Qualified candidates hold a Bachelor's in nursing, a Virginia RN license, and 3 years of RN experience.

Qualifications

• Bachelor’s degree in nursing is required.
• Three years of RN experience is required.
• Virginia RN license and BLS certification are required.

Responsibilities

• New patient intake and pre-visit planning: obtain records, identify missing data, and coordinate completion before visit.
• Coordinate ILD diagnostic workup per protocol (HRCT, PFTs, DLCO, 6MWT, ABG when indicated).
• Monitor symptoms, oxygen needs, imaging and lab results; ensure timely follow-up and escalation when needed.
• Educate patients on ILD diagnosis, disease course, triggers, vaccines, energy conservation, oxygen safety, pulmonary rehab expectations.
• Facilitate antifibrotic therapies, prior authorizations, and patient assistance resources.
• Coordinate home oxygen initiation or titration and arrange DME; ensure payer documentation.
• Triage calls for dyspnea, cough, fever, desaturation; provide protocol-based guidance and escalate appropriately.
• Coordinate referrals for rehab, smoking cessation, nutrition, social work and palliative care.
• Identify transplant referral indicators and assemble records for consideration when appropriate.
• Document care plans and outcomes in EMR; communicate with referring providers and teams.
• Support clinic quality initiatives and workflow improvements.
