Inova Health System in Virginia seeks a dedicated Transplant Coordinator – Post Heart to join our team. This full-time role includes regular office hours, Monday–Friday, with on-call duties (approximately 3 weekends per year and 2 weekday nights monthly) and remote triage support.

The ideal candidate is an active RN with a Bachelor’s degree in nursing, Virginia licensure, and BLS certification, with at least three years of RN experience.

Qualifications

• RN experience required.
• Bachelor’s degree in nursing or equivalent.
• Virginia RN license and BLS certification required.

Responsabilités

• Continually assesses patient needs and collaborates with health-care team.
• Completes documentation of care plans and orders per policy.
• Educates patients, families and care providers; documents education.
• Organizes data to ensure regulatory compliance and accurate reporting.
• Participates in quality improvement initiatives and program assessment.
