Job summary

Darke County Recovery Services is seeking a qualified Service Coordinator/Case Manager to support clients with SPMI/PMI in a collaborative setting. You will coordinate care, document promptly, and work with supervisors to implement treatment plans.

The role requires a 4-year degree or higher and 3+ years of relevant experience, or an Associate's degree with less experience. A valid driver’s license and adequate auto insurance are required.

Qualifications

  • Bachelor's degree or higher is preferred with 3+ years working with SPMI/PMI populations.
  • Associate's degree in Social Services or Psychology may be considered with less experience.
  • Valid driver's license and auto insurance (100,000/300,000 liability) are required or may be required for transportation.

Responsibilities

  • Inform clients of scope of practice and supervision status.
  • Perform limited assessment of client needs; help develop and implement treatment plans.
  • Coordinate with a multidisciplinary team and provide case management.
  • Maintain confidential clinical documentation within 24 hours of services.
  • Engage in community outreach and education activities as assigned.

All employees are eligible to participate in our 403B retirement program.ResponsibilitiesInforms clients of scope of practice, Therapeutic Behavioral Support [TBS] or Psychiatric Social Rehabilitation [PSR], and describes supervision status, including name and credential of supervisorPerforms limited assessment of client needs. TBS will provide comprehensive treatment planning. PSR will implement the comprehensive treatment plan. Service Coordination/Case management service delivery is provided in collaboration with clients and supervisorCoordinates interventions with a multi-disciplinary teamDevelops a professional trusting relationship with clients and familiesHelps clients obtain needed material resourcesProvides support to clients during crisis situationsHelps clients learn and use coping skills necessary to meet demands of community living in order to move toward greater self-relianceHelps motivate clients to engage in the community and become involved in community sponsored social activitiesProvides education and support to client’s family members and significant others regarding client’s diagnosis, mental health condition, and how they can assist client to meet treatment goalsAdvocates for the rights and dignity of the client in the communityMaintains efficient clinical documentation within 24 hours of services providedParticipates in the assessment of program needs and peer review, as well as cooperate with performance improvement objectivesComplies with supervision requirements and works within responsible scope of practicePerforms community outreach and education assignments including mental health education and prevention programs, as well as consultation services with supervisionProvides at least a minimum of 50% productivity or more than 90 hours of billable services monthlyComplies fully with confidentiality and protected health information rulesAttends staff meetings, in-services, case conferences, supervision, etc., as scheduledPerforms other duties as assigned including support activities as needed to augment functioning of the agencyCollaborates in maintaining clean, safe workplace, including maintenance of CPR/First Aid skills and aggressive client management skills (TAPS or equivalent)Productivity Expectation: 100%RequirementsRequired Credentials and SkillsA 4 year degree or higher from an accredited institution and 3 or more years of experience working with the SPMI/PMI population is preferred. An Associate’s degree in a Social Services or Psychology related field, with less than 3 years experience, may be considered.Driver’s license and appropriate automobile insurance of 100,000/300,000 liability. Service Coordinator/Case Manager needs to have clarification from insurance agent that the Service Coordinator/Case Manager, on rare occasions MAY have to transport clients in his / her personal vehicle.Supervision available “in-house” from appropriate supervisory staffOral and written communication skillsDisplays good customer service skills with clients and community at largeCompetency in areas prerequisite for performing activities noted above with supervisionUtilizes computer well enough to perform clinical documentation in CareLogic (MIS system) directly, as well as generate written documentation in WORD or via emailPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee must have a valid driver’s license to drive a motor vehicle and a safe driving record.
