Job summary

Valley Comprehensive Community Mental Health Center Inc in Morgantown, WV is seeking a full-time Case Manager in the I/DD division. The role focuses on linking clients with services, coordinating supports, and ensuring informed choice within annual budgets.

You will work under the Program Manager of Mental Health Care Coordination & IDD Case Management, use clear communication, complete IPPs, and advocate for client rights.

Qualifications

• BA/BS in Human Services or related field OR 1 year experience working with I/DD.
• Ability to comply with Valley’s policies and procedures.
• Ability to read, write, understand, and speak the English language.
• Valid Driver’s License.
• Current CPR/First Aid certificate or ability to be trained.

Responsibilities

• Use clear communication with clients and other professionals.
• Assist the person/legal representative with re-determination of financial eligibility as required at the DHHR office.
• Begin the discharge process and provide linkage to services appropriate to the level of need.
• Provide information about the agency’s rights and grievance procedures.
• Assist with procurement of services for IPP and annual medical evaluations as applicable.
• Act as an advocate for the person and coordinate with related systems.
• Provide education, linkage, and referral to community resources.
• Promote a meaningful social role for the person in the community.
• Interface with UMC regarding assessment, purchase of services, and IPP budgeting.
• Coordinate IPPs annually and ensure IPP documentation is uploaded within required timeframes.
• Ensure safety concerns are addressed and reporting requirements are met.
