Job summary

Seneca Health Services in West Virginia seeks an IDDW Case Manager to coordinate supports for individuals with developmental disabilities, aligning with client goals and community inclusion.

You will collaborate with the IDT to develop IPPs, monitor services, and ensure compliance with state requirements while safeguarding the client’s health, safety, and rights. This role supports documentation and referrals for community resources.

Responsibilities

• Assist the client and/or legal representative with re-determination of financial eligibility at the DHHR office.
• Provide education, linkage and referral to community resources and ensure freedom of choice with regard to service type and service provider.
• Coordinate necessary evaluations to be utilized as a basis of need and recommendation for services in the development of the IPP.
• Document all services, both paid and unpaid, from any and all programs on the IPP.
• Monitor to ensure the client’s health and safety needs are addressed.
