Job summary

YAI is seeking a qualified professional to collaborate with DDS and Regional Centers on intake and referral processes for individuals with IDD and behavioral health needs.

The role emphasizes leading intake meetings, developing IPPs and person-centered plans, coordinating transition plans, and performing risk assessments with senior clinical leadership. Monthly visits or more may be required throughout transitions.

Qualifications

  • Master’s degree in Psychology, Social Work, or Counseling with 5+ years' experience with IDD and behavioral/mental health.
  • Extensive experience in developing and implementing IPPs and person-centered plans for IDD with criminal/forensic involvement.
  • Experience linking IDD clients with complex behavioral/mental health needs to community supports.

Responsibilities

  • Collaborate with DDS and Regional Centers on the Intake and Referral Process.
  • Facilitate and lead Intake/admission meeting.
  • Collaborate with the Clinical Coordinator, Consultants, and other IDT members on IPP, Person Centered Plan, and transition plans.
  • Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director.
  • Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan.
  • Support the person pre-transition, during transition and post-transition with at a minimum of monthly visits or more frequently as necessary.
  • Performs all other duties, as assigned.
