You will build relationships through ongoing case management, advocacy, and coordination of services in community settings. The role emphasizes collaboration, documentation, and professional development, with a schedule based in Luzerne County, PA, offering remote flexibility.

Qualifications

• Strong interpersonal, organizational and communication abilities.
• Ability to work independently and manage a dynamic caseload.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess individual strengths, needs and life goals.
• Develop person-centered plans with the individual and their circle of support.
• Coordinate services to meet goals and ensure timely access to supports.
