Job summary

Bluebonnet Trails Community MHMR Center is seeking a Service Coordinator to assess needs, develop service plans, link to services, and monitor effectiveness for individuals with developmental disabilities.

The role includes crisis intervention, advocacy, and collaboration with families and BTCS staff across agencies, with travel within Caldwell and Guadalupe Counties and flexible hours.

Qualifications

• An associate degree or higher in social, behavioral, or human service field.
• Or a high school diploma with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities.
• Must meet Bluebonnet Trails driving requirements and maintain approved driving status.
• Preferred Qualifications: At least one year of experience working with individuals with intellectual/developmental disabilities.
• Bachelor’s Degree.
• Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting.
• Demonstrated computer and documentation skills.
• Knowledge of appropriate community resources.
• Bilingual (English/Spanish/ASL).
• Experience with Medicaid programs.

Responsibilities

• Completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage.
• Monitoring and advocating on behalf of individuals served.
• Interaction with the individual/family served.
• Travel within the service area and flexible hours and schedule.
