Bluebonnet Trails Community Services is seeking a passionate, self-motivated Habilitation Coordinator to join our team. This rewarding position serves as a vital advocate for adults with intellectual and developmental disabilities (IDD) and suspected mental illness who reside in, or are at risk of admission to, nursing facilities.
As a Habilitation Coordinator, you will play a critical role in helping vulnerable individuals access the services and supports they need to thrive. You will complete intake, assess needs, coordinate specialized services, advocate for community living opportunities, and support individuals whether they remain in a nursing facility or decide to transition into the community.
This position offers a unique opportunity to combine case management, advocacy, assessment, service coordination, and transition planning while making a meaningful impact in the lives of others.

Hourly Salary Range: $21.46 – $28.71

A bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention.
At least one year experience working with individuals with Intellectual and Developmental disabilities is required, this experience can be paid or unpaid.
