Job summary

The Service Coordinator position at Bluebonnet Trails Community Services supports children and adults with developmental disabilities using Person-Centered Thinking. You assess needs, develop service plans, facilitate linkages, and monitor effectiveness while advocating for individuals and ensuring services meet Medicaid/HHSC/MCO guidelines.

Travel within the service area is required; positions require flexible hours and driving.

Qualifications

• Associate degree or higher in social, behavioral, or human service field.
• High school diploma with two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
• Must meet driving requirements and maintain approved driving status.

Responsibilities

• Assess individual needs and desires to inform service planning.
• Develop and monitor service plans and linkages to appropriate supports.
• Advocate for individuals and coordinate with families and providers.
• Participate in interagency planning and CRCG meetings to bridge gaps in services.
