Job summary

OneWell Health Care in Dayton, OH is seeking an IDD Program Specialist to coordinate, monitor, and evaluate services for individuals with IDD to ensure person-centered, high-quality care. The role collaborates with families, County Boards of Developmental Disabilities, DSPs, and providers to implement ISPs and ensure regulatory compliance.

You will promote independence, community inclusion, and positive outcomes while maintaining thorough documentation and adherence to Ohio DODD and HCBS

Qualifications

• Bachelor's degree in a related field is required.
• Minimum of two years experience supporting individuals with IDD.
• Experience with case management, program coordination, or service delivery.
• Experience with Ohio Medicaid Waiver programs is preferred.
• Supervisory or leadership experience is preferred.
• Experience working with County Boards of Developmental Disabilities is a plus.

Responsibilities

• Coordinate, monitor, and evaluate services for individuals with IDD to ensure person-centered, high-quality care.
• Collaborate with individuals, families, County Boards of Developmental Disabilities, DSPs, and community providers to implement ISPs.
• Ensure regulatory compliance and promote independence, community integration, and positive outcomes.
• Provide guidance and oversight to Direct Support Professionals.
• Maintain accurate documentation and regulatory compliance.
• Promote participant rights and community inclusion.
