Job summary

HELP Foundation, Inc. is seeking a Program Specialist in Ohio to ensure residential services meet high standards. The role involves leading team coordination, data collection on interventions, and ensuring informed consent is documented. On-site responsibility with a focus on client needs and ISP goals.

Qualifications

• High school diploma or GED required; a 4-year degree preferred.
• At least 1 year of experience in Developmental Disabilities.
• Experience interpreting and implementing consumer ISP goals.
• At least 2 years of experience using and reviewing service documents and records.
• Knowledge of provider licensure rules.
• Proficient in Microsoft Office.
• Strong time management and organization skills.
• Excellent verbal and written communication; on-call ability.
• Must possess valid Ohio Driver's License with <6 points.
• Must pass background check including BCI and FBI.

Responsibilities

• Prepares consumer program documentation, including skill development plans and behavior support documents.
• Lead contact for the team with SSA’s.
• Conducts client assessments and collects data on interventions.
• Assures informed consent is assessed and documented.
• Attends client team meetings and negotiates staffing needs.
• Trains staff in delivery of consumer service plans and communicates changes.
• Organizes and maintains client files.
• Participates in regular meetings reviewing client needs and quality of life.
• Involves team members when consumer needs change.
• Investigates incidents and develops prevention plans with managers.
• Drives personal vehicle between assignments and transports clients.
