Job summary

Extended Home Care of NY is seeking an experienced Occupational Therapist to provide home-based therapy for IDD patients in the Bronx, Manhattan, Queens, and NY areas. You will evaluate and treat patients per physician plans, develop goals, and document progress while coordinating with the rehab team.

The role requires NY state OT licensure, a graduated OT program, and at least two years of institutional or home care experience. Competitive pay and comprehensive benefits are offered.

Qualifications

• Licensed OT in New York State required.
• Graduate of an OT program approved by AOTA/ACME.
• Minimum two years institutional or home care experience required.
• Must satisfy state health status requirements.

Responsibilities

• Assess OT needs using objective tests and determine therapy status.
• Prepare initial assessments and periodic re-assessments.
• Develop and document the plan of care with measurable goals.
• Provide OT treatment according to physician's plan and collaborate with physician.
• Document patient response and adjust care plan as needed.
• Instruct patient, family and caregivers on OT techniques for safe mobility.
