Job summary

Extended Home Care of NY is seeking a Mandarin or Cantonese speaking occupational therapist to conduct home visits in the Bronx for IDD patients. The OT provides therapy per a physician's plan of care, assessing functional status and contributing to the total plan of care.

Responsibilities include assessments, developing and adjusting care plans, documenting progress, and educating patients and families. Requires NYS OT license and 2 years’ experience in home care or institutional settings.

Qualifications

• Mandarin or Cantonese language proficiency required.
• Degree from a baccalaureate or master's program in physical therapy (as stated in ad).
• Graduate of an Occupational Therapy program approved by relevant associations.
• License and current registration to practice as a Registered Occupational Therapist in NYS.
• Two years institutional and/or home care experience required.
• Satisfy state health status requirements.

Responsibilities

• Assess OT needs using objective diagnostic/prognostic tests.
• Prepare initial assessments and periodic re-assessments.
• Develop plan of care per physician prescription with estimated treatment duration.
• Define services, frequency of visits, and measurable goals.
• Provide OT treatment in collaboration with patient’s physician.
• Document response to treatment and modify plan as needed.
• Instruct patient, family on safe mobility and restoration techniques.
• Complete case conferences, 60-day and discharge summaries.
