Job summary

Qualifications

• At least 1 year of experience as a staff Occupational Therapist; 2+ years preferred in acute care.

Responsibilities

• Evaluate patient’s functional level at home and work with patient/family to provide training or assistive devices.
• Collaborate with health team to establish rehabilitation goals and activities.
• Document patient progress, treatments, and discharge summaries.
• Communicate with Agency Nurse every two weeks to discuss progress and any changes in care.
