In Home Rehab Group is seeking an Occupational Therapist to join our in-home treatment program in New York. You will deliver patient-centered OT services in clients' homes, focusing on functional tasks and daily living activities.

Responsibilities include creating individualized treatment plans, conducting assessments, documenting progress, and coordinating care with families and healthcare providers. Strong communication and ability to work independently and as part of a team are essential.

Formación

• Bachelor's or Master's degree in Occupational Therapy required.
• Current New York State licensure as an Occupational Therapist required.
• Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
• Excellent communication and interpersonal skills.
• Ability to work effectively independently and as part of a team.

Responsabilidades

• Provide occupational therapy services to patients in their homes, focusing on functional skills and daily living activities.
• Develop and implement individualized treatment plans, in collaboration with patients, families, and healthcare providers.
• Conduct thorough assessments and evaluations to identify patients' needs and goals.
• Utilize a variety of treatment techniques and strategies to promote patient progress and independence.
• Maintain accurate and timely documentation of patient progress and treatment plans.
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
• Participate in team meetings and professional development activities to stay current with best practices and industry developments.
