Job summary

Extended Home Care of NY is seeking a Mandarin or Cantonese-speaking Occupational Therapist to conduct home visits in the Bronx for IDD patients. The OT will provide therapy per physician plans, assess functional status, and contribute to the total care plan.

Responsibilities include initial assessments, ongoing re-evaluations, and collaboration with the Interdisciplinary Team. The role requires NYS OT licensure, a degree in occupational or physical therapy, and at least two years in

Qualifications

• Mandarin or Cantonese language proficiency required.
• Degree in physical therapy from an approved program.
• New York State license to practice as an OT.
• Two years institutional and/or home care experience required.
• Satisfy the state health status requirements.

Responsibilities

• Assess OT needs using objective diagnostic and prognostic tests.
• Prepare initial assessment and periodic re-assessments.
• Develop care plan with prescribed therapy services and estimated visit details.
• Provide therapy services to meet written physician plans and goals.
• Document responses and adjust treatment with physician notification.
• Instruct patient, family, and caregivers in safe mobility techniques.
• Complete case conferences, 60-day, transfer and discharge summaries as indicated.
• Submit weekly visit notes or within 7 days after each home visit.
