Job summary

RHA Health Services, LLC seeks an IDD Nurse RN to provide holistic nursing care and oversight across residential homes, vocational centers, and schools. You will evaluate health needs, coordinate preventive care, document care, and mentor licensed practical nurses and med techs as needed.

Qualifications

• Graduation from an accredited registered nurse program; BSN preferred but not required.
• Current RN license with no restrictions to practice in Illinois.
• CPR certification or eligibility to obtain certification.
• Able to communicate effectively with interdisciplinary team members, the people supported and their families.
• Able to work independently, be organized, and prioritize tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, assess, and evaluate the health needs of the people we support.
• Coordinate preventative and routine medical care for people supported.
• Document nursing care provided.
• Provide clinical supervision or oversight for LPNs and med techs as deemed necessary.
• Assist with educational and training programs for new employees.
• Promote continuous improvement of workplace safety and environmental practices.
• Assist physicians with on-site medical clinics where applicable.
• Provide health-related training to unlicensed personnel.
• Teach medication administration training to unlicensed personnel when state law allows.
• Conduct medication pass observations in the home to ensure staff competency.
• Complete skill competency checklists on delegated healthcare tasks.
• Teach the people we serve to monitor their health.
• Provide medication administration training to people who can self-administer.
• Audits medication administration.
• Conduct medication closest assessments.
• Perform quality assurance checks to ensure unlicensed personnel are performing health tasks as assigned.
• Conduct routine nursing assessments.
• Observe and report abnormal health findings to the physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for the people supported and assist with developing community supports.
• Participate in on-call rotations to provide nursing support.
