Job summary

RHA Health Services, LLC is hiring an IDD Nurse (LPN) to provide holistic nursing care in residential and vocational settings. The role reports to the Regional Nursing Director and requires graduation from an accredited licensed nurse program with an active LPN license and CPR certification.

Responsibilities include health assessments, coordinating medical care, documenting nursing care, and on-site clinics.

Qualifications

Graduate from an accredited licensed nurse program.
Current LPN license with no restrictions to practice nursing.
Current CPR certification or eligibility to certify for CPR.
Possess credentials required by Federal or State law.
Effective communication with Interdisciplinary Team Members and families.

Responsibilities

Provide holistic nursing care to individuals with intellectual and developmental disabilities.
Observe and gather health data, report needs to RN.
Coordinate preventive and routine medical care.
Document nursing care provided.
Assist physicians with on-site medical clinics where applicable.
Provide health-related trainings to unlicensed staff.
Conduct medication pass observations and closest assessments.
Perform quality checks to ensure staff perform health tasks correctly.
Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
Advocate for people supported and promote community health.
