Job summary

RHA Health Services, LLC is seeking an IDD Facility Nurse (RN) for Walnut Creek, CA on a PRN basis. You will provide holistic nursing oversight for individuals with IDD, mental health, and substance use needs, promoting health and independence.

Responsibilities include medication administration, health assessments, staff training, collaborating with physicians and the IDT, and advocating for client well-being. Requires RN or LPN, active license, and CPR certification.

Qualifications

• Graduate of an accredited LPN OR RN program.
• Active, unrestricted nursing license.
• Strong communication with interdisciplinary team, individuals served, and families.
• Ability to work independently, stay organized, and prioritize tasks.
• Current CPR certification or willingness to obtain.

Responsibilities

• Provide holistic nursing care to individuals with IDD.
• Observe, assess, and evaluate health needs of individuals.
• Deliver health-related training to unlicensed personnel.
• Educate individuals on monitoring their health and medication administration when appropriate.
• Administer medications and treatments, or assist with self-administration.
• Ensure preventative and routine medical appointments are scheduled and maintained.
• Conduct medication closet assessments.
• Perform quality assurance to ensure health-related tasks are completed correctly.
• Collect and document health-related data.
• Conduct comprehensive nursing assessments.
• Monitor for abnormal findings and report to physicians promptly.
• Collaborate with physicians, pharmacies, medical suppliers, and IDT.
• Advocate for individuals and develop community supports to promote health and independence.
