Corewell Health is seeking an experienced registered nurse to join the Adult Intellectual and Developmental Disabilities Clinic in Grand Rapids. You will educate, follow up with patients and families, and provide emotional support while case managing across inpatient and outpatient settings to maximize continuity of care.

Qualifications

• Bachelor's Degree Nursing is required.
• Master's Degree in Nursing is preferred.
• Minimum 5 years of nursing or CDE clinical experience is required.
• Experience teaching patients of all ages is required.
• MI RN licensure is preferred; state licensure is required where applicable.

Responsibilities

• Provide quality education, follow-up, and emotional support to patients and families.
• Case manage the population across inpatient and outpatient settings to maximize continuity of care.
• Manage projects and programs related to clinical management and quality improvement.
