Corewell Health is seeking an experienced registered nurse to join the Adult Intellectual and Developmental Disabilities Clinic in Grand Rapids. You will educate, follow up with patients and families, and provide emotional support while case managing across inpatient and outpatient settings to maximize continuity of care.

The role emphasizes quality improvement, collaboration with multidisciplinary teams, and a day shift schedule in a full-time capacity.

Qualifications

• Bachelor's Degree Nursing is required.
• Master's Degree in Nursing is preferred.
• Minimum 5 years of nursing or CDE clinical experience is required.
• Experience teaching patients of all ages is required.
• MI RN licensure is preferred; state licensure is required where applicable.

About Outpatient Clinics

Scope of Work

• Responsible for providing quality education, follow-up and emotional support to the population and their families.
• Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings.
• Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

Qualifications

• Bachelor's Degree Nursing (Required)
• Master's Degree Nursing (Preferred)
• 5 years of previous nursing or CDE clinical experience (Required)
• Prior teaching with patients of all ages (Required)
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan (Preferred)
• Upon Hire required CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required
• CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 120 Days required
• CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 120 Days required
