Corewell Health's Adult Intellectual and Developmental Disabilities Clinic - Medical Group West in Grand Rapids is seeking a full-time nurse educator/case manager. You will educate patients and families, provide follow-up and emotional support in both inpatient and outpatient settings.

The role emphasizes continuity of care, quality improvement, and collaboration with multidisciplinary teams. Day shift, 40 hours weekly, 8:00 a.m. – 5:00 p.m., Monday through Friday at the Beltline site.

Qualifications

• Bachelor’s Degree Nursing is required.
• 5 years of nursing or clinical experience required.
• RN license in Michigan upon hire.
• BLS/ACLS/PALS certifications within specified timelines.

Responsibilities

• Provide quality education, follow-up and emotional support to patients and families.
• Case manage across inpatient and outpatient settings to maximize continuity of care.
• Manage projects and quality improvement activities related to clinical management.

About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Scope of work

Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

Qualifications

• Required Bachelor’s Degree Nursing
• Preferred Master’s Degree Nursing
• 5 years of previous nursing or CDE clinical experience Required
• Prior teaching with patients of all ages Preferred
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 120 Days required or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 120 Days required
