Job summary

Aspire Community Services is seeking a Housing Case Manager to support adults with Intellectual and Developmental Disabilities in obtaining and maintaining safe, stable housing. You will apply a Whole Person-Centered Care approach to empower independence and community inclusion.

Responsibilities include assessments, housing plans, coordinating applications and benefits, and working with Supports Coordinators and providers to advocate for tenants and ensure housing stability.

Qualifications

  • Must be 18 years or older.
  • Valid driver’s license and reliable transportation.
  • Ability to travel throughout the community for participant visits.
  • Background clearances required.
  • Experience with IDD or human services preferred.

Aspire Community Services (ACS) is seeking a compassionate, organized, and motivated Housing Case Manager to support adults with Intellectual and Developmental Disabilities (IDD) in obtaining and maintaining safe, stable housing. Through our Whole Person-Centered Care approach, you'll empower individuals to achieve greater independence, community inclusion, and an improved quality of life.

What You’ll Do
  • Conduct comprehensive assessments to identify housing needs, strengths, barriers, and goals.
  • Develop individualized housing service plans using a Whole Person-Centered Care approach.
  • Assist individuals with IDD in locating and maintaining safe, affordable, and accessible housing.
  • Coordinate housing applications, rental assistance, public benefits, and community resources.
  • Collaborate with Supports Coordinators, healthcare providers, behavioral health professionals, and community agencies.
  • Advocate for participants with landlords, housing authorities, and service providers.
  • Build positive relationships with landlords to increase housing opportunities.
  • Provide education on budgeting, tenant rights, independent living skills, and community integration.
  • Conduct home visits to monitor participant well-being, housing stability, and progress toward goals.
  • Complete quarterly home safety reviews, participate in fire drills, and ensure homes remain safe and compliant with agency and regulatory standards.
  • Respond appropriately to housing crises and emergency situations.
  • Maintain accurate electronic documentation, assessments, case notes, referrals, and progress reports.
  • Attend staff meetings, trainings, and professional development activities.
  • Follow all agency policies, HIPAA regulations, ODP requirements, and documentation standards.
Qualifications
Required
  • Must be at least 18 years of age.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
  • Ability to travel throughout the community for participant visits.
  • Successfully pass all required background clearances.
  • Strong communication, organization, advocacy, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficient with computers and electronic documentation systems.
  • Commitment to Whole Person-Centered Care and supporting individuals with IDD.
Preferred
  • Experience working with individuals with Intellectual and Developmental Disabilities (IDD).
  • Knowledge of Pennsylvania ODP services, waiver programs, housing resources, and community supports.
  • Experience in case management, housing services, behavioral health, or human services.
Education & Experience

One of the following is preferred:

  • High School Diploma or GED with 3+ years of relevant experience.
  • Associate's Degree (or 60 college credits) in Human Services, Social Work, Psychology, Sociology, Criminal Justice, or a related field with 2+ years of experience.
  • Bachelor's Degree in a related field with 1+ year of relevant experience.
  • Master's Degree in a related field preferred.
Physical Requirements
  • Ability to sit, stand, bend, and walk for extended periods.
  • Ability to lift up to 25 pounds occasionally.
  • Frequent travel to participant homes, community appointments, and meetings.
  • Ability to safely operate a motor vehicle.
