• Experience in caregiving, home health, or direct support for individuals with IDD is preferred.
• Ability to use electronic documentation and follow care plans.

Responsibilities

• Provide consistent, respectful support per care plan.
• Assist with personal care and daily living activities.
• Support meal preparation, nutrition, mobility, and medication assistance as authorized.
• Implement behavior support strategies.
• Monitor health, safety, and emotional well-being.
• Promote independence and community participation by maintaining routines and transportation assistance.
• Participate in trainings and team meetings; maintain timely documentation.
