Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with Intellectual and Developmental Disabilities in home and community settings in Pennsylvania. The role emphasizes daily living assistance, health and safety monitoring, and community inclusion while promoting independence and dignity.

The DSP will follow individualized service plans under supervision, support transportation to appointments and community activities,

Qualifications

• High school diploma or equivalent is required or equivalent.
• Experience in caregiving, home health, or direct support is preferred.
• Proficiency in written and spoken English is required.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies under supervision of program staff.
• Maintain accurate documentation and timely notes, with GPS verification where applicable.
• Support transportation to appointments, community activities, and employment/vocational activities.
• Participate in trainings, team meetings, and adapt to staffing needs while complying with regulations.
