Job summary

Camino Real Community Services in Floresville, TX seeks a Community Liaison (Case Manager) to promote awareness of services for individuals with IDD experiencing a mental health event. You will collaborate with the service planning team to develop crisis respite plans and support the transition from crisis to stable supports.

The role requires a Bachelor's degree in a related field, QMHP qualifications, and strong documentation and communication skills.

Qualifications

• Bachelor’s Degree in a relevant field or a registered nurse.
• QMHP qualifications as per TAC requirements.
• Minimum 2 years working with IDD or linking IDD to mental health supports, plus 1 year required experience.
• Strong documentation and timely reporting skills.
• Strong customer relations and professional communication; able to work in teams.

Responsibilities

• Promote awareness of services for people with IDD experiencing a mental health event.
• Assess for crisis and develop crisis respite service plans.
• Collaborate with the service planning team on follow-up and relapse prevention.
• Educate Camino Real staff and the community on engaging individuals with IDD.
• Monitor crisis respite services and program outcomes; track forms and reports.
