Qualifications

• Bachelor's degree in Human Services field.
• At least one year of experience working with people with developmental disabilities.
• Experience in residential settings preferred.

Responsibilities

• Provide case management for residents in group homes, coordinating services.
• Develop and monitor client plans, complete essential documentation.
• Maintain up-to-date client and group home files.
• Plan and participate in recreational, therapeutic, and training activities.
• Communicate effectively with the IDT and supervisor.
• Report any suspected abuse or neglect as required by policy.
