Job summary

InAlliance in Sacramento, CA is seeking an experienced human services professional to coordinate and implement individualized support plans for participants with IDD, ensuring safety and quality of care. The role collaborates with circles of support, healthcare providers, and staff to deliver comprehensive services and advocacy.

Requirements include a high school diploma, 3+ years in human services (IDD preferred), 2+ years direct support, and proficiency with MS Office.

Qualifications

• Must have a minimum of three years’ experience working in the Human Services field, preferably in services for individuals with Intellectual and Developmental Disabilities (IDD).
• At least two years of direct support experience with adults with IDD.
• Proficiency in MS Office Suite.

Responsibilities

• Develop and implement person-centered Individualized Support Plans (ISP) in collaboration with participants and their circle of support.
• Conduct assessments to identify participants’ needs, preferences, and goals.
• Monitor and adjust support plans regularly to ensure they meet participants' evolving needs.
• Coordinate and facilitate access to community resources, healthcare services, and other supports for participants.
• Advocate for participants' rights and needs within the community and service systems.
• Provide coaching, mentoring, and performance feedback to direct support staff to ensure high-quality service delivery.
• Develop staff schedules and ensure adequate coverage for participant support.
