Job summary

Qualifications

• Bachelor's degree in human services or related field preferred.
• At least one year of experience with IDD population.
• Valid driver's license and current automobile insurance.
• Ability to pass registry and background checks.

Responsibilities

• Provide habilitative, behavioral and health care coordination services to clients with IDD as part of a multi-disciplinary ISP team.
• Complete timely and accurate documentation within Therap and other documentation as required of the position.
• Assess, plan, implement, and document care and habilitative coordination services to assigned clients.
• Ensure ISP expectations are fulfilled and the ISP is correct; inform appropriate personnel of ISP expectations.
• Complete regular file reviews to ensure compliance with the ISP and regulatory requirements.
