Job summary

Mosaic Beatrice seeks a dedicated Habilitative Coordinator-QIDP/Case Manager to empower individuals, coordinate services, and ensure regulatory compliance in Beatrice. You will oversee diverse caseloads, lead annual reviews, and collaborate with families, supervisors, and coordinators to promote growth and independence.

The role emphasizes staff training in behavior modification, development of innovative service methods, and rigorous documentation to maintain high-quality care across programs.

Qualifications

• Bachelor’s degree in psychology, special education, or related field.
• At least 3 years hands-on experience with individuals with intellectual/developmental disabilities.
• QIDP certification (or state requirements), Med Aide 40, CPR/First Aid, Mandt certification.
• Deep knowledge of habilitative services and state regulations.

Responsibilities

• Oversee a caseload of up to 22 individuals, creating programs that support growth and independence.
• Lead annual meetings and assessments (CFA, POM, SIS) and present before the Human Rights Committee.
• Collaborate with Home Supervisors, families, guardians, and Service Coordinators to drive admissions and ensure regulatory compliance.
• Train direct care staff in behavior modification techniques and help develop service methodologies.
• Maintain high standards and clear, professional documentation.
