Responsibilities

• Provide habilitative, behavioral and health care coordination services to clients with IDD as part of a multi-disciplinary ISP team.
• Complete timely and accurate documentation within Therap and other required documentation.
• Assess, plan, implement, and document care and habilitative coordination services to assigned clients.
• Ensure the ISP expectations are fulfilled and the ISP is correct; notify relevant personnel.
• Conduct regular file reviews to ensure ISP/regulatory compliance.
• Engage in interdisciplinary collaboration to address client needs and referrals.
