Job summary

Independent Living Systems (ILS) in Daytona Beach, FL seeks a Care Coordinator (IDD) to join our team. The role focuses on assessing needs, developing person-centered plans, coordinating services, and advocating for members to ensure quality of life and independence.

You will collaborate with healthcare providers, social workers, families, and community resources, monitoring progress and adjusting care plans while ensuring compliance with guidelines and program requirements.

Qualifications

• Two years of experience working with individuals with intellectual and developmental disabilities.
• Bachelor’s degree or equivalent education in a social services field.
• RN or LPN license to practice in Florida.
• Proficiency with EHR systems and basic computer applications.

Responsibilities

• Assess member needs and identify care gaps to develop person-centered support plans.
• Coordinate services and supports across care settings to ensure continuity of care.
• Monitor progress and adjust care plans as needed to achieve optimal outcomes.
• Serve as liaison among members, families, providers and community resources.
• Provide education and support on available resources and self-advocacy.
• Maintain accurate documentation and comply with policies and quality standards.

Education

Bachelor's degree in social work, sociology, psychology, gerontology, or related social services field

Bachelor's degree in a field other than social science

RN licensed to practice in Florida

LPN with minimum 4 years of relevant experience (IDD)

Certification/licensure related to care coordination (CCM/CCRC)
