Job summary

Independent Living Systems (ILS) is seeking a Care Coordinator (IDD) to join our team. The role focuses on managing and coordinating comprehensive care for eligible individuals with intellectual and developmental disabilities, ensuring access to person-centered services that promote health, independence, and quality of life.

You will assess needs, develop support plans, coordinate services, monitor progress, and advocate for members, families, and providers while adhering to clinical guidelines

Qualifications

• Minimum of two years of relevant experience with individuals with intellectual developmental disabilities.
• Degree(s) as listed: social work/sociology/psychology/gerontology or related field; RN/LPN licensure.
• Proficiency with EHR and basic computer applications.

Responsibilities

• Serve as primary contact for the member and authorized representatives.
• Assess member needs and develop a person-centered support plan.
• Monitor member’s progress and adjust care plans as necessary.
• Coordinate services across the continuum and communicate with providers and community resources.
• Provide education and support on available resources and self-advocacy.
• Maintain documentation and ensure compliance with policies, regulations, and quality standards.

Registered Nurse (RN) licensed in Florida
